Dr. Connor Rhodes fights to operate on Pamela, a young woman with a heart infection that is causing mini-strokes. Dr. Daniel Charles, acting as the Chief of Psychiatry, overrides the patient's consent for surgery, arguing she is not mentally competent to make the decision. Sharon Goodwin eventually sides with Rhodes, allowing the surgery to proceed, which creates a significant rift between Rhodes and Charles.

The central story follows Sharon Goodwin as she reunites with her first love, Reggie Dixon, who arrives at the hospital in critical condition. Sharon allows her emotions to take the lead, interjecting herself into the treatment plan led by Dr. Choi and Dr. Manning. Despite their diagnosis of a long-dormant parasite and their warnings that Reggie will not survive, Sharon pushes for extraordinary measures like ECMO.
